At the oral proceedings, they further stated that the arrangement of the axes in the present case resulted in a presentation of the ECG data according to the Cabrera system (well-known to physicians) which illustrates ECG leads in an anatomically more meaningful manner.
It is used to diagnose acute myocardial infarction in the presence of left bundle branch block (LBBB). In the presence of other clinical findings Cabrera's sign suggests the presence of acute myocardial infarction. Spodick sign: Downsloping TP segment. ECG changes associated with Stage I pericarditis, first described by David H. Spodick in 1974.

THE CASE.
Cabrera's sign refers to a notch that occurs within the first 40 msec in the ascending portion of the S wave in lead V3 or V4: Cabrera's sign (arrowed) This ECG sign was first described by Cabrera & Friedland in 1953. Although Cabrera's sign has a high specificity for the diagnosis of an old myocardial infarction in LBBB, it has a poor sensitivity.
The 12-lead system includes three different types of leads: bipolar, augmented or unipolar, and precordial. Each of the 12 standard leads presents a different perspective of the heart’s electrical activity; producing ECG waveforms in which the P waves, QRS complex, and T Cabrera sign indicating old myocardial infarction the knowledge of specific signs on surface ECG could prove the opposite. The Cabrera sequence makes it easy to understand that ST-segment elevation in lead –aVR during anterolateral AMI reflects an extensive area at risk . 15 We previously reported that in patients with anterolateral AMI who underwent successful recanalization within 6 h of symptom onset, ST-segment depression in lead aVR on the admission ECG was

Cabrera sign (kă-brā′ră) Electrocardiographic evidence of myocardial infarction in patients with left bundle branch block or ventricular pacing, in which there is a 0.04 second notch in the ascending limb of the S wave of leads V3, V4, or V5.
The hexaxial reference system, better known as the Cabrera system, is a convention to present the extremity leads of the 12 lead electrocardiogram, that provides an illustrative logical sequence that helps interpretation of the ECG, especially to determine the heart's electrical axis in the frontal plane. Cabrera sign is defined as the presence of a prominent, shelf-like or downward notching, of >0.04 second duration, at the ascending limb of the S wave. Cabrera and Friedland in 1953 found that late notching of the up-slope of the S wave in leads V3–V5, is highly sensitive and specific for the diagnosis of old MI in LBBB. In the total population, the Cabrera sign presented the highest specificity (97%) and diagnostic accuracy (62.2%), with a sensitivity of 23.6%.

The QTc interval is within normal range. Also note the absence of Q waves or T-wave inversion that would be indicative of ECG evolution after ischemic ST-segment elevation.
